First Year Out Plan
The P.E.L.F. Program offers a comprehensive First Year Out Plan designed to guide individuals on a clear path to success after their release.
How It Works:
1. Goal Setting
• Immediately after release, a P.E.L.F. staff member will meet with the individual to create a personalized list of goals.
2. Identifying Needs
• Together, we’ll determine what is needed to achieve these goals, such as employment, transportation, clothing, housing, or other resources.
3. Creating a Plan
• We will prioritize the goals and break them into manageable steps to make progress achievable.
4. Providing Support
• P.E.L.F. staff will offer resources, guidance, and 24/7 support to help individuals stay on track and overcome obstacles.
5. Weekly Progress Check-Ins
• Regular weekly sessions will be held to review progress, address challenges, and identify any additional support needed.
With P.E.L.F., no one has to navigate this journey alone. We’re committed to helping individuals build a strong foundation for a successful transition back into society.
